Help pls, I am looking for the recipe of a casserole that has chicken, mushrooms and leeks in it?

My ex used to make this dish, I am not on speaking terms with her ,can someone who has got a clue pls give me a clue.

Just chunk cut your boneless chicken breast and throw it in a casserole dish with a can of mushroom soup. Add some leeks, potatoes, chicken stock, carrots, peas, etc. Bake it at 400 degrees for 30 minutes. Take it out and sprinkle on some bread crumbs and bake for an additonal 15 minutes.
